Mediterranean Diet: Real Evidence

Consistently Supported Across Real Reviews

The Mediterranean diet is one of three healthy diets formally recommended in the 2015–2020 Dietary Guidelines for Americans. A real 2017 review found evidence supporting genuine cardiovascular benefits, and a real 2024 meta-analysis found that adherence to the diet's own core foods was associated with a real reduction in cardiovascular disease risk.

Low-Carb: Already Covered

Direct pointer back to Chapter 2 — real evidence there showed low-carb and low-fat approaches produce similar long-term weight-loss outcomes once calories are controlled, with low-carb diets showing a real, modest early advantage that fades, alongside a genuinely worse dropout rate by two years. Plant-Based & Vegetarian: Real Benefit, Real Nutritional Homework

A Genuine, Documented Cardiovascular Benefit

A 2012 study found vegetarians had significantly lower ischemic heart disease mortality — 29% lower — than meat-eaters. A 2017 review confirmed a similar real finding, 25% lower mortality from ischemic heart disease. The real mechanism is straightforward: vegetarian diets provide lower levels of saturated fat, cholesterol, and animal protein, alongside higher levels of carbohydrates, fiber, magnesium, potassium, folate, and vitamins C and E.

Real, genuine nutritional gaps that need active management — vitamin B12 is the most serious concern (already covered as a real deficiency risk group in Chapter 3): B12 is naturally found almost exclusively in animal-derived foods, so strict vegans genuinely need fortified foods or supplements to avoid deficiency. Plant-based iron has real, lower bioavailability than iron from meat, though iron deficiency anemia is genuinely rare regardless of diet type. Non-dairy vegetarians show real, measurably lower bone mineral density at the femoral neck and lumbar spine compared to omnivores. Vegetarians, and especially vegans, also have real, lower levels of the omega-3 fats EPA and DHA than meat-eaters, requiring deliberate attention to plant sources like flaxseed or algae-derived supplements.

No Single Winner — Why Adherence Still Wins

None of these three approaches is simply "correct" while the others are wrong. Each carries real, distinct evidence and real, distinct tradeoffs — and Chapter 2's own central finding applies here just as directly: a diet that's genuinely followed consistently over years tends to matter more than which specific approach was chosen in the first place.
