This chapter covers the least settled ground in this course so far. The macronutrient and micronutrient chapters rested on decades of accumulated evidence; gut microbiome research is genuinely newer, faster-moving, and — outside one well-proven exception covered directly below — still mostly correlational rather than causal.

A Real, Corrected Number

The "10x more bacteria than human cells" claim is outdated — a widely repeated figure for years, it was genuinely revised by a 2016 study (Sender et al.). The real, current estimate: roughly 38 trillion bacterial cells in the human body, against roughly 30 to 40 trillion human cells — approximately the same order of magnitude, not a 10-to-1 ratio.

What Fiber Actually Does There

Some gut microorganisms genuinely benefit their host by fermenting dietary fiber into short-chain fatty acids (SCFAs) — including acetic acid and butyric acid — which the body then absorbs. This is a real, established mechanism, not a speculative one, and it directly ties this chapter back to Chapter 2's own coverage of fiber as a distinct carbohydrate subtype.

A Real, Proven Success Story: FMT for C. diff

Fecal Microbiota Transplantation

FMT is genuinely, strongly effective against recurrent Clostridioides difficile infection — approximately 85–90% effective in people for whom antibiotics haven't worked or whose infection keeps recurring after treatment. A landmark 2013 randomized controlled trial found 81% of patients cured after a single infusion, and over 90% cured after a second. The FDA has approved three real FMT products — Rebyota (November 2022) and Vowst (April 2023, the first oral formulation) — and permits broader use for recurrent C. diff specifically under an enforcement discretion policy, provided informed consent and real donor screening are in place. The FDA has also issued genuine safety alerts after cases of multi-drug-resistant organism transmission from inadequately screened donors.

Why This Field Still Deserves Real Caution

FMT's own real success for C. diff is a narrow, specific, and genuinely proven case — not a general endorsement of "gut health" interventions broadly. Beyond that specific use, the field's own real research base largely consists of studies showing an association between the microbiome and various conditions, without yet establishing a proven causal mechanism in most cases. Microbiome-based treatment more generally remains, in real terms, still in its infancy.

The Honest Takeaway

Two real things are true at once: fiber's role in producing beneficial short-chain fatty acids is genuinely established science, and FMT for recurrent C. diff is a genuinely proven, FDA-recognized treatment — while most broader claims about probiotics, "gut health," and the microbiome's role in other conditions remain real, active research questions rather than settled answers.
